• I certify that I am the owner or authorized agent for the cat listed above and am authorized to make healthcare decisions for this patient.
• I authorize the veterinarians and staff of Midwest Feline Medical Center to perform the physical examination and routine non-invasive procedures necessary to evaluate my cat.
• I understand that additional diagnostics, treatments, medications, hospitalization, sedation, anesthesia, or procedures will be discussed with me before being performed whenever reasonably possible.

Photo / media permission *

• Payment is due at the time services are provided unless another arrangement has been approved in advance.
• An estimate may be provided for recommended diagnostics or treatment. If an estimate is not provided, I can ask for one.
• Medical needs can change during evaluation or treatment and actual costs may differ from an initial estimate.
• I am financially responsible for services I authorize.
